My name is Geny Zapata and I am a doctoral candidate in the Psy.D. Program at the University of La Verne.  I am conducting a research study which seeks to understand resilience and spirituality as coping factors when measured with levels of pain suffering.

Individuals who are eligible to participate in this study are adults 18-80 years of age and have either a Type I or Type II, Complex Regional Pain Syndrome diagnosis.

It will take approximately 30-40 minutes to complete this questionnaire.  You will be able to stop, save your answers, and return to finish the questionnaire at a later time if you need to, using the same computer by clicking on the “Next” button.  This will save the last page you filled out so that you can return to the questionnaire at a later time.  Demographic, medical history, and psychological information will be collected/assessed as part of the survey.  You will not be asked to provide any contact information and your answers on this questionnaire will be kept strictly confidential.  Your participation will be greatly appreciated.  The information under consideration will benefit psychological research and the medical communities by providing further understanding and knowledge on the importance of resilience and spirituality as coping factors in the treatment of CRPS.

As a way of demonstrating my appreciation for your participation in completing the questionnaire, I will have a raffle where twenty participants will receive a $25.00 gift certificate to Borders Book Store.  Participation in the raffle is optional.  In order to be entered into the raffle, you must be willing to provide your e-mail address to the researcher.  Your e-mail address will be separated from your answers and will not be used for any other purpose than to inform you if you have won one of the raffle prizes.  Further instructions on the raffle will be provided at the end of the questionnaire.
